GPAI Article 55 Incident Report Template
Article 55(1)(c) requires providers of GPAI models with systemic risk to report serious incidents to the AI Office. This template lays out the notification structure aligned with the GPAI Code of Practice expectations.
Notification structure
- Provider + model identification (name + version + cumulative training FLOPs).
- Date of awareness + date of incident.
- Nature of the serious incident: capability misuse, safety failure, biosecurity, cybersecurity, manipulation.
- Estimated scale of harm.
- Containment measures taken (model access revoked / pulled, capability filters tightened, downstream notification cascade).
- Root-cause analysis findings.
- Mitigations: model retraining, RLHF round, capability filter update, deprecation.
- Downstream-deployer cascade (per Article 53 documentation hand-down).
