Notification structure

• Provider + model identification (name + version + cumulative training FLOPs).
• Date of awareness + date of incident.
• Nature of the serious incident: capability misuse, safety failure, biosecurity, cybersecurity, manipulation.
• Estimated scale of harm.
• Containment measures taken (model access revoked / pulled, capability filters tightened, downstream notification cascade).
• Root-cause analysis findings.
• Mitigations: model retraining, RLHF round, capability filter update, deprecation.
• Downstream-deployer cascade (per Article 53 documentation hand-down).
